Information we collect: 

Name, partner/spouse name, email address, phone number, mailing address, birth date, and emergency contact information.  Members may voluntarily provide information regarding their health, personal service needs, etc. For our volunteers, we also collect driver's license and car insurance information.  New volunteers are requested to complete a background check through a third-party service. Any online payments (membership dues, donations) are processed through a third-party service (Stripe), AAH does not retain, or have access to,  payment information like credit card numbers. 

How we collect personal information: 

Members provide this information as a part of a written membership agreement.  Volunteers provide this information as part of a written Volunteer Registration Agreement.

Why we need this information and how we use it:

We need address information to provide volunteer services onsite at the member's location, contact information is used to schedule and confirm requests and appointments.  

How we share this information:

Contact information is shared between members and volunteers in order to enable the fulfillment of service requests.  Some additional personal information, such as mobility needs, my be provide when appropriate.  Other personal information, such as birth dates, driver license and insurance information, etc., is not shared.  All information that is shared is subject to our confidentiality agreement.
